A horse with sweet itch can rub out an entire mane, damage the tail, open the skin and become increasingly distressed despite fly sprays, rugs and topical treatments.

Omega-3 supplements are often promoted as a natural solution because of their potential anti-inflammatory effects. Flaxseed is the most commonly recommended option for horses, but the evidence is considerably less definitive than many supplement labels suggest.

Omega-3s may provide useful nutritional support for some affected horses. They do not stop Culicoides midges from biting, and they should not be relied upon as the main treatment for an actively itchy horse.

Flaxseed provides ALA rather than substantial amounts of preformed EPA or DHA.

Horses appear capable of converting at least some ALA into longer-chain fatty acids, but direct dietary EPA and DHA are incorporated into blood and muscle more reliably than ALA from flaxseed. What Does the Research Actually Show?

The 2002 milled-flaxseed study

The University of Guelph-associated study most commonly used to support flaxseed for sweet itch involved only six horses that had positive skin-test reactions to Culicoides extract.

The trial was:

  • Double-blinded

  • Placebo-controlled

  • Crossover in design

  • Conducted over 42-day supplementation periods

  • Based primarily on reactions to intradermal allergen testing

The horses received approximately 1 pound of milled flaxseed for every 1,000 pounds of body weight each day. This equates to approximately 0.1 percent of body weight, or roughly 500 grams daily for a 500-kilogram horse.

At day 42, the Culicoides skin-test reaction was smaller while horses were receiving flaxseed than while they were receiving the bran placebo. No significant difference was found at day 21. (PMC)

What the study did not prove

The study did not demonstrate that flaxseed:

  • Stopped naturally occurring sweet itch

  • Prevented midges from biting

  • Eliminated clinical itching

  • Healed open skin lesions

  • Replaced medication

  • Worked in a large population of affected horses

It measured the size of an experimentally induced skin-test reaction in a very small number of horses.

The 1999 linseed-oil study

A separate double-blinded crossover trial studied 17 horses with clinical seasonal Culicoides hypersensitivity.

The horses received either:

  • 200 mL of linseed oil daily, or

  • 200 mL of corn oil daily

Each treatment period lasted six weeks.

The researchers measured the number of itching behaviours and the total area of skin lesions. Linseed oil did not produce a statistically significant improvement in either clinical itching or lesion area compared with corn oil, although many owners believed their horses looked better during linseed supplementation. This difference between owner impression and objective measurements is important. Coat shine or general appearance may improve without the allergic disease itself being adequately controlled.

The topical omega-3 cream study

A 2019 study evaluated a topical cream containing concentrated fish oil, moisturisers and other ingredients in horses with Culicoides hypersensitivity.

The treated side developed significantly greater improvement in visible skin lesions than the untreated side. However, overall itching scores did not improve significantly, and five horses experienced adverse effects. Is Ground Flaxseed Better Than Flaxseed Oil?

Ground flaxseed provides:

Antibiotics should only be used when secondary bacterial infection is suspected or confirmed. They do not treat the underlying allergy.

Omega-3s are biologically plausible and may support equine skin health, but the evidence for treating sweet itch remains limited and inconsistent.

One tiny flaxseed study found a reduction in an experimentally induced skin reaction after six weeks. A larger linseed-oil study found no significant improvement in natural clinical itching or lesion area. That is not enough evidence to promise owners that flaxseed will control Culicoides hypersensitivity.

The strongest treatment remains preventing bites.

Use physical barriers, appropriate turnout management, fans, screening and repellents. Treat damaged or infected skin early. Use veterinary anti-inflammatory medication when the horse’s comfort and welfare require it.

Flaxseed can be considered as one supportive part of that plan. It should never be the plan itself.
