A guinea pig that gradually becomes reluctant to walk, struggles to chew or starts losing weight may have more than arthritis or ordinary dental disease.
Fibrous osteodystrophy, commonly called Satin Syndrome when it occurs in Satin-coated guinea pigs, causes widespread abnormal bone remodelling. What Is Satin Syndrome?
Satin Syndrome is the informal name given to fibrous osteodystrophy associated with the Satin coat phenotype.
Fibrous osteodystrophy occurs when normal mineralised bone is excessively resorbed and partly replaced by weaker fibrous connective tissue. The skeleton can become:

Is Satin Syndrome the Same as Hyperparathyroidism?
Fibrous osteodystrophy is consistent with excessive parathyroid hormone activity, but the reason for that activity is uncertain.
Parathyroid hormone helps maintain blood calcium. When the body senses insufficient available calcium, or when tissues fail to respond normally to the hormone, more calcium may be removed from the skeleton.
Several possible mechanisms have been proposed.
Nutritional Secondary Hyperparathyroidism
Two non-Satin guinea pigs with fibrous osteodystrophy were suspected of having nutritional secondary hyperparathyroidism caused by an unsuitable calcium-to-phosphorus balance. Renal Secondary Hyperparathyroidism
Kidney abnormalities have been found in some affected Satin guinea pigs, including interstitial nephritis, pyelonephritis, nephrocalcinosis and altered renal structures. Pseudohypoparathyroidism-Like Disease
One affected Satin guinea pig had markedly elevated parathyroid hormone despite normal ionised calcium, normal phosphorus and no clear evidence of renal failure. This remains a hypothesis based on one detailed case. A causative genetic mutation has not been demonstrated in Satin guinea pigs.
A Genetic Disorder
The strong association with the Satin phenotype and the characteristic pattern of disease make an inherited mechanism likely, but the exact genetic relationship remains unknown.

Satin Syndrome appears to involve abnormal calcium and bone regulation, but it cannot currently be reduced to one proven dietary, renal or genetic mechanism.
Does Every Satin Guinea Pig Develop the Disease?
No.
In a study of 52 Satin guinea pigs, 20 were clinically affected while 32 had no obvious clinical abnormalities at the time of assessment. However, this was a selected clinical population rather than a random population study, so the result should not be interpreted as a true 38% lifetime risk for every Satin guinea pig. (HERO)

Clinical disease is often recognised in young adults, including guinea pigs between approximately one and two years old. However, it is not restricted to guinea pigs under two.
In the radiographic study, older Satin guinea pigs had significantly more severe skeletal scores than younger animals. This supports the view that disease may begin early and progress with age. No significant difference was identified between males and females. (Refubium)

Vitamin C deficiency is especially important because guinea pigs cannot synthesise vitamin C. Long-term low vitamin C intake has been shown experimentally to reduce total bone mineral density and bone mineral content. (PubMed)
The mistake I would avoid is diagnosing Satin Syndrome solely because a shiny-coated guinea pig becomes lame. Radiographs and a complete dental, nutritional and medical assessment are needed.

Satin Syndrome is not proven to result from simple dietary calcium deficiency.

Treating Only the Teeth
Dental overgrowth may be secondary to jaw disease. Repeated trimming without pain control and skeletal assessment misses the wider condition.
Assuming Normal Bloodwork Rules It Out
Blood calcium and renal values may be within reference limits despite advanced radiographic disease.
Forcing Exercise
A painful or osteopenic guinea pig may fracture or become more distressed.
Breeding an Affected Satin Guinea Pig
A genetic mechanism is strongly suspected even though the exact mutation has not been identified. From a welfare perspective, avoiding breeding affected animals and close relatives from heavily affected lines is prudent.
