An itchy horse with crusty skin, hair loss and constant rubbing may have mange, but mites are not the only possible explanation. Pastern dermatitis, lice, fungal infection, allergies and chronic progressive lymphedema can all produce similar signs.

The location of the lesions matters. Severe itching around heavily feathered lower legs strongly suggests chorioptic mange, while lesions around the head, neck, mane, ears or shoulders may point towards a different mite or another skin condition entirely.

The mistake I see most often is treating every crusty leg with random washes before confirming what is actually causing the problem. That can delay the correct treatment, irritate already damaged skin and allow contagious mites to spread through the yard.

What Is Mange in Horses?

Mange is a skin disease caused by microscopic mites.

Depending on the mite involved, they may:

  • Live on the surface of the skin

  • Feed on skin debris

  • Burrow into superficial skin layers

  • Live inside hair follicles or sebaceous glands

  • Trigger a severe allergic reaction despite relatively small numbers

What Types of Mange Affect Horses?

What Is Chorioptic Mange?

Chorioptic mange is caused by Chorioptes bovis, historically called Chorioptes equi. It is often referred to as leg mange or feather mites.

These mites live on the surface of the skin and feed on skin debris rather than burrowing deeply. Despite this superficial feeding behaviour, sensitive horses can develop severe itching and inflammation. (Merck Veterinary Manual)

Common signs include:

  • Repeated foot stamping

  • Kicking at the stable wall

  • Biting the lower legs

  • Rubbing one leg against another

  • Scales beneath the feathers

  • Small papules or bumps

  • Matted hair

  • Crusts and scabs

  • Thickened or wrinkled skin

  • Moist, inflamed pastern dermatitis

  • Secondary bacterial infection

Can a horse carry mites without itching?

Yes.

Some horses carry substantial numbers of chorioptic mites without showing obvious irritation. Other horses react severely to relatively small mite populations.

A horse that looks normal can therefore act as a source of reinfestation for more sensitive horses in the same group.

What Is Psoroptic Mange?

Psoroptic mange is caused by mites within the Psoroptes group.

It is rare in many regions, but when it occurs it can cause marked itching and lesions around:

  • The forelock

  • The mane

  • The base of the tail

  • Beneath the chin

  • Between the hind legs

  • The udder

  • The axillae

  • The ears

The lesions may begin as small papules and areas of hair loss before progressing to thick, sometimes blood-stained crusts.

When the ear canals are affected, the horse may show:

  • Head shaking

  • Ear sensitivity

  • Resistance to the bridle

  • Brown crusty material within the ears

  • Swelling or odour around the ear base

What Is Sarcoptic Mange?

Sarcoptic mange is caused by a horse-adapted form of Sarcoptes scabiei. It is rare, but it is generally considered the most severe form of equine mange.

The mites burrow within the superficial layers of the skin and trigger an intense hypersensitivity response. Lesions commonly begin around the:

  • Head

  • Neck

  • Shoulders

  • Sides

  • Back

The lower limbs and areas protected by long hair may initially be less affected.

Signs can include:

  • Sudden, extreme itching

  • Constant rubbing and scratching

  • Small bumps or vesicles

  • Dry crusts

  • Widespread hair loss

  • Thickened, folded or wrinkled skin

  • Bleeding from self-trauma

  • Reduced appetite

What Is Demodectic Mange?

Equine demodicosis is caused by Demodex equi or Demodex caballi. These mites live within hair follicles and sebaceous glands.

Unlike chorioptic, psoroptic and sarcoptic mange, demodectic mange is usually not intensely itchy.

Possible signs include:

  • Patchy hair loss

  • Scaling

  • Small skin nodules

  • Lesions around the eyelids or muzzle

  • Lesions over the neck, shoulders or forelimbs

Clinical demodicosis is extremely rare in horses. Reported cases have sometimes occurred alongside immune compromise, pituitary pars intermedia dysfunction or prolonged corticosteroid treatment. Is Mange Contagious Between Horses?

Chorioptic, psoroptic and sarcoptic mites can spread between horses.

Transmission may occur through:

  • Direct horse-to-horse contact

  • Shared grooming brushes

  • Shared rugs or blankets

  • Boots and bandages

  • Halters and lead ropes

  • Stable partitions

  • Bedding

  • Trailers

  • Shared areas used for rolling or rubbing

  • Contaminated hands and clothing

Chorioptic mites can survive away from the horse under suitable conditions, although estimates vary. Environmental survival for days or weeks is enough to make contaminated equipment and housing important sources of reinfestation.

When one horse in a close-contact group is confirmed with chorioptic mange, the others should be considered potentially exposed. That does not mean every horse must automatically receive the same medication, but all in-contact horses should be examined and included in the veterinary control plan.

Can People Catch Mange From Horses?

It depends on the mite.

Chorioptic mange

Horse-associated chorioptic mites are not known to cause established infestation in people. (wcvm)

Sarcoptic mange

Animal-adapted Sarcoptes mites can cause temporary itchy papules in people who handle an infested horse. This has historically been called “cavalryman’s itch”.

The mites generally do not reproduce successfully on a person who is not their preferred host, but the skin reaction can still be very uncomfortable.

Anyone developing an itchy rash after contact with a suspected sarcoptic case should:

  • Avoid further unprotected contact

  • Wash exposed clothing

  • Tell their doctor about the horse exposure

  • Follow the veterinarian’s biosecurity instructions

A human rash should be diagnosed by a human medical professional rather than treated using the horse’s medication.

Why Are Feathered Horses at Greater Risk?

Chorioptic mange is particularly important in breeds with dense lower-leg feathering, including:

  • Shires

  • Clydesdales

  • Belgian draft horses

  • Gypsy horses and traditional cobs

  • Friesians

  • Percherons

  • Other heavily feathered draft types

Dense feathers can retain warmth, moisture, skin debris and crusting while making early lesions difficult to see.

A 2025 Belgian study found chorioptic mites in 144 of 156 Belgian draft horses examined, compared with none of the 142 Belgian Warmbloods in that particular study. This should not be interpreted as the prevalence in every country or breed, but it demonstrates how heavily affected some draft-horse populations can be. (Springer Link)

Is Mange the Same as Chronic Progressive Lymphedema?

No.

Chronic progressive lymphedema, or CPL, is a lifelong disorder involving impaired lymphatic drainage in the lower limbs. It causes progressive swelling, thick skin folds, nodules and fibrosis.

Chorioptic mange does not explain the entire condition. However, mites, bacterial infections and chronic inflammation can aggravate CPL lesions and make the horse considerably more uncomfortable.

Treating mites may improve itching and some skin lesions, but it does not cure the underlying lymphatic disease. (Center for Equine Health)

Signs suggesting CPL rather than uncomplicated mange include:

  • Persistent lower-limb swelling

  • Thick skin folds around the pastern

  • Firm nodules beneath the feathers

  • Progressive enlargement of the lower legs

  • Deep moist crevices

  • Foul-smelling discharge

  • Reduced flexibility of the skin

  • Increasing difficulty moving comfortably

A feathered horse with recurring “mites” despite repeated treatment should be examined specifically for CPL and other causes of chronic pastern dermatitis.

What Else Can Look Like Mange?

Not every itchy, crusty horse has mites.

Important alternatives include:

Pastern dermatitis

“Greasy heel”, “scratches” and “mud fever” describe a clinical pattern rather than one specific disease. Causes can include moisture damage, bacterial infection, mites, contact irritation, photosensitivity, vasculitis and chronic lymphedema.

Lice

Lice can cause itching, rubbing, rough hair, weight loss and visible eggs attached to the hairs. They are especially common during colder months.

Dermatophilosis

Dermatophilus infection can produce tightly adherent crusts with matted tufts of hair, particularly after prolonged wet weather.

Ringworm

Dermatophyte infection usually causes circular or irregular areas of scaling and hair loss. It may be mildly itchy and is contagious to other animals and people.

Insect-bite hypersensitivity

Culicoides allergy and other insect reactions commonly affect the mane, tail, midline and face and may be strongly seasonal.

Pinworms

Pinworm irritation commonly causes rubbing around the tail head and anus rather than crusting confined to the lower legs.

Contact or chemical irritation

Harsh shampoos, disinfectants, concentrated essential oils and incorrectly diluted insecticides can create skin lesions that resemble the original disease.

Autoimmune disease, vasculitis or photosensitisation

These are less common but important considerations when lesions are painful, ulcerated, unusually distributed or unresponsive to routine treatment.

The location, degree of itching, season, breed and whether other horses are affected all help narrow the list, but they rarely provide a complete diagnosis alone.

How Do Veterinarians Diagnose Mange?

Diagnosis begins with the history and lesion distribution.

Your veterinarian may ask:

  • When the itching began

  • Whether signs are seasonal

  • Which body areas were affected first

  • Whether another horse is itchy

  • Whether new horses recently arrived

  • Which rugs and equipment are shared

  • Which products have already been applied

  • Whether the horse has PPID, CPL or another chronic disease

  • Whether the horse has received corticosteroids

Skin scrapings

Chorioptic and psoroptic mites usually live near the skin surface, so superficial scrapings are taken from several active areas.

Samples may also be collected using:

  • Hair and coat brushings

  • Adhesive tape

  • Crusts and surface debris

  • Material from around the coronary band

  • Ear debris when ear mites are suspected

Multiple sites may need to be sampled because mites are not distributed evenly. Recovering an adult mite, egg or recognisable mite fragment confirms the infestation. (Merck Veterinary Manual)

Deeper testing

Depending on the presentation, your veterinarian may also recommend:

  • Deeper skin scrapings

  • Skin cytology

  • Bacterial culture

  • Fungal testing

  • Skin biopsy

  • Blood testing

  • PPID testing

  • Examination for CPL

  • Investigation of systemic disease

A secondary bacterial infection can continue causing pain and discharge even after the mite burden has been reduced.

How Worried Should You Be?

Low Risk

The horse has:

  • Mild intermittent foot stamping

  • Limited scaling under the feathers

  • No open wounds

  • No swelling or lameness

  • Normal appetite and behaviour

  • No other affected horses

What to do: Separate grooming equipment, inspect all four legs and arrange a routine veterinary examination within several days.

Moderate Risk

The horse has:

  • Persistent itching

  • Repeated biting or rubbing

  • Expanding crusts or hair loss

  • Moist pastern dermatitis

  • More than one affected horse

  • Recurring winter flares

  • Skin thickening beneath heavy feathers

What to do: Restrict shared equipment and contact your veterinarian promptly. Diagnostic sampling and a coordinated treatment plan are needed.

High Risk

The horse has:

  • Extensive raw or bleeding skin

  • Marked swelling

  • Pain or lameness

  • Foul-smelling discharge

  • Rapidly spreading lesions

  • Reduced appetite

  • Weight loss

  • Severe CPL

  • Failure of several previous treatments

What to do: Arrange same-day veterinary assessment. Secondary cellulitis, deep infection or an incorrect original diagnosis may be present.

Critical

The horse develops:

  • Fever with a severely swollen limb

  • Inability or unwillingness to bear weight

  • Rapidly progressive painful swelling

  • Collapse or profound weakness

  • Severe widespread skin sloughing

  • Facial swelling or breathing difficulty after treatment

  • Neurological signs following application of an insecticide

  • Severe self-trauma that cannot be controlled safely

What to do: Treat this as an emergency and contact an equine veterinarian immediately.

When Is Mange an Emergency?

Mange itself is usually a chronic dermatological condition rather than a sudden life-threatening emergency.

Urgent treatment is required when complications develop.

Contact your veterinarian immediately if the horse has:

  • Fever

  • Rapidly increasing limb swelling

  • Severe pain

  • Non-weight-bearing lameness

  • Deep cracks or ulceration

  • Pus or foul-smelling discharge

  • Redness spreading up the limb

  • Marked lethargy

  • Loss of appetite

  • Severe weight loss

  • Eye involvement

  • Collapse, tremors or incoordination after treatment

  • Breathing difficulty or widespread swelling after medication

A hot, painful, swollen leg with fever may represent cellulitis or lymphangitis rather than uncomplicated mites. Do not keep applying mite products while waiting for it to settle.

What Should You Do Right Now?

1. Separate equipment immediately

Stop sharing:

  • Brushes

  • Combs

  • Rugs

  • Boots

  • Bandages

  • Towels

  • Halters

  • Lead ropes

  • Clippers

Label equipment for the affected horse.

2. Limit unnecessary contact

Do not move the horse through several communal stables or grooming areas.

Complete isolation may not always be practical, particularly when the entire group has already been exposed, but unnecessary direct contact and equipment sharing should stop while your veterinarian assesses the situation.

3. Inspect the entire horse

Check:

  • All four lower limbs

  • Beneath the feathers

  • The tail head

  • Mane and forelock

  • Ears

  • Chin and axillae

  • Between the hind legs

  • Ventral abdomen

Wear gloves when handling crusts and wash your hands afterwards.

4. Record the lesions

Take clear photographs before applying medication.

Also record:

  • Date signs began

  • Areas affected

  • Degree of itching

  • Foot stamping frequency

  • Other affected horses

  • Previous treatments

  • Any recent arrivals

Do not deliberately provoke the horse into scratching merely to obtain a video.

5. Do not pick off dry crusts

Pulling firmly attached crusts from inflamed skin can cause bleeding, pain and further barrier damage.

Crusts may need to be softened and removed gradually as part of the veterinary treatment plan.

6. Avoid random home remedies

Do not apply:

  • Diesel or kerosene

  • Bleach

  • Undiluted disinfectant

  • Concentrated essential oils

  • Livestock dip mixed by guesswork

  • Dog mite products

  • Human scabies cream

  • Unlabelled agricultural chemicals

A product that kills mites can also damage a horse, particularly when applied to raw skin or used at the wrong concentration.

How Is Mange Treated in Horses?

There is no single treatment protocol that is ideal for every horse or every mite.

The plan depends on:

  • The mite species

  • Lesion location

  • Severity

  • Number of exposed horses

  • Presence of feathers

  • Secondary infection

  • Country-specific product approvals

  • Competition regulations

  • Previous treatment response

  • Underlying CPL or immune disease

1. Improve Access to the Skin

Topical treatment must reach the skin rather than sitting on top of several centimetres of dense hair.

In heavily feathered horses, careful clipping can:

  • Improve examination

  • Expose hidden lesions

  • Allow medication to contact the skin

  • Make cleaning easier

  • Reduce retained moisture and debris

Clipping is not a cure by itself. It must also be performed carefully when the skin is thickened, folded, painful or ulcerated.

A veterinarian may recommend sedation for safe clipping when the lower legs are severely painful.

2. Remove Excess Debris Gently

Crusts and compacted debris can reduce topical treatment penetration.

Depending on the condition of the skin, the treatment plan may include:

  • Gentle washing

  • A keratolytic shampoo

  • Soaking of crusts

  • Careful drying

Aggressive scrubbing can worsen inflammation and create additional infection.

Products used or investigated for equine mange include:

  • Lime sulfur preparations

  • Certain labelled pyrethrin or permethrin products

  • Veterinarian-directed moxidectin

  • Other extra-label topical protocols in selected cases

Product registration varies between countries. Even when the active ingredient is familiar, the formulation, concentration and route of administration may make a major difference.

Merck Veterinary Manual notes that lime sulfur is labelled for equine mange in the United States and that some permethrin sprays carry equine mange claims. Mite eggs and immature stages may survive the first application. Repeated treatment is usually timed to interrupt the life cycle as new mites hatch.

Antibiotics do not kill mites, and mite medication does not automatically resolve a deep bacterial infection.

  2. Clean away hair, crusts, dirt and organic debris.

  3. Wash reusable equipment.

  4. Apply only disinfectants or insecticides appropriate for that surface and purpose.

  7. Keep affected and unaffected equipment separate.

  8. Clean trailers and grooming areas.

  9. Replace or safely discard equipment that cannot be cleaned.

Simply spraying disinfectant over dirty bedding is unlikely to solve the problem.

7. Recheck the Horse

Reduced itching does not necessarily prove every mite has been eliminated.

A follow-up examination may include:

  • Repeating skin scrapings

  • Checking in-contact horses

  • Reviewing lesion photographs

  • Assessing skin healing

  • Investigating persistent swelling

  • Changing the treatment protocol

  • Looking for CPL or another underlying disease

Chorioptic mange is notorious for partial improvement followed by recurrence.

What Does Recent Treatment Research Show?

Research has produced encouraging results with several protocols, but no treatment should be copied directly from a paper without veterinary oversight.

In a small 2023 controlled study, a high-dose cattle moxidectin pour-on formulation was applied topically to clipped lower legs of Belgian draft horses. No living mites were detected in treated horses at day 14, and itching and skin lesions improved. However, mites were again found in three horses by the 24-week assessment. (ScienceDirect)

This study demonstrates that effective mite reduction can improve welfare and CPL-associated skin lesions. It does not turn cattle pour-on into a routine owner-administered horse treatment. The regimen was extra-label, used a specific concentration and dose, and was performed under research supervision.

A Critical Medication Warning

Do not assume that a mite treatment labelled for cattle, sheep, dogs or another species is safe for horses.

AAEP external parasite guidance specifically warns:

Do not use amitraz on horses.

Improvised application of concentrated insecticides can cause:

  • Colic

  • Reduced intestinal movement

  • Depression

  • Weakness

  • Incoordination

  • Skin burns

  • Respiratory distress

  • Collapse

Always check the exact product, active ingredient, concentration, species label and dilution before applying anything.

How Long Does Mange Take to Clear?

Some horses become less itchy within several days of effective treatment, but the skin may take weeks to heal.

Recovery depends on:

  • How long the infestation has been present

  • Mite species

  • Skin damage

  • Secondary infection

  • Presence of CPL

  • Whether all contacts were addressed

  • Environmental control

  • Whether the medication reached the skin

  • Whether repeat treatments were completed

Hair regrowth and resolution of thickened skin generally lag behind mite control.

A horse should be reassessed if:

  • Itching has not clearly improved after the expected treatment period

  • Environmental contamination contributes to reinfestation

  • Some medications reduce but do not eliminate mites

  • The horse may have concurrent CPL

Common Mistakes Horse Owners Make

Assuming every crusty pastern is mud fever

Mites, bacterial infection, CPL and immune-mediated disease can all produce similar lesions.

Treating only the most obviously itchy horse

A comfortable-looking in-contact horse may still carry chorioptic mites.

Applying one treatment and stopping

One application may reduce adult mites without interrupting the complete life cycle.

Leaving thick feathers and crusts untouched

Topical medication cannot work properly if it never reaches the skin.

Scrubbing until the skin bleeds

More aggressive cleaning is not more effective. It may worsen infection and inflammation.

Using an oral wormer without a veterinary plan

Ivermectin and moxidectin may be used in some protocols, but ordinary deworming does not guarantee elimination of chorioptic mites.

Ignoring the stable and equipment

A treated horse can be exposed again through contaminated grooming equipment, bedding or close-contact horses.

Using livestock chemicals by guesswork

Assuming improvement means cure

How Can Mange Be Prevented?

Complete prevention is not always possible, particularly in heavily feathered groups, but risk can be reduced.

Inspect new horses before introduction

Pay close attention to:

  • Fetlocks and pasterns

  • Tail head

  • Mane

  • Ears

  • Areas of scaling beneath feathers

  • Foot stamping or leg biting

A new horse with unexplained itching or crusts should not immediately share grooming equipment or close housing with the resident group.

Quarantine higher-risk arrivals

During quarantine:

  • Use dedicated brushes and equipment

  • Record skin changes

  • Watch for foot stamping
