Gastric dilatation and volvulus (GDV)—commonly known as bloat—is one of the most devastating and life-threatening emergencies in large-breed dogs. Thankfully, a simple surgery called prophylactic gastropexy can help prevent it. 🩺🐾

In this guide, I’ll explain what gastropexy is, which dogs need it, and how it dramatically lowers the risk of a fatal GDV event. 🐶🔁

⚠️ What Is GDV (Bloat)?

GDV occurs when a dog’s stomach fills with gas or food and then twists on itself. This rotation traps gas inside the stomach, compresses blood vessels, and leads to shock, organ failure, and death within hours without surgery.

Signs of GDV:

  • Distended abdomen
  • Non-productive retching
  • Pacing, restlessness
  • Excessive drooling
  • Rapid breathing or collapse

💔 Without immediate emergency surgery, dogs with GDV will not survive.

🔧 What Is Prophylactic Gastropexy?

Gastropexy is a preventative surgery that tacks the stomach to the abdominal wall, preventing it from twisting if it fills with gas. It doesn’t stop the stomach from expanding—but it greatly reduces the chance of twisting.

📉 Risk of fatal GDV drops to 6% with gastropexy vs. up to 40% lifetime risk in some breeds without it.

🐕 Dogs at Highest Risk

GDV can occur in any breed, but risk is highest in large, deep-chested dogs, including:

  • Great Danes (up to 40% lifetime risk)
  • St. Bernards
  • Weimaraners
  • German Shepherds
  • Boxers
  • Irish Setters
  • Standard Poodles

🧠 Anxiety is also a contributing risk factor for GDV.

🩺 When to Consider Gastropexy

The procedure can be done:

  • ✅ At the time of spay or neuter (ideal)
  • ✅ Electively in young dogs over 6 months
  • ✅ After a GDV emergency to prevent recurrence

🔬 Types of Gastropexy Surgery

Several surgical approaches are available:

✂️ Traditional Open Surgery

  • Two incisions made—one on the body wall, one on the stomach
  • Stomach sutured in place for permanent tacking

🔍 Laparoscopic Gastropexy

  • Minimally invasive, faster recovery
  • Uses a camera and small instruments
  • Ideal for working or performance dogs

📹 Endoscopic-Assisted Gastropexy

  • Even less invasive—access via the mouth and esophagus
  • Used in specialty hospitals with advanced equipment

🏥 Recovery After Preventative Surgery

Recovery is similar to spay surgery:

  • 🛏️ Keep calm and leashed for 7–10 days
  • 🚫 No running, jumping, or rough play
  • 🧼 Keep the incision clean and dry

🚨 Emergency GDV Surgery Recovery

More intensive aftercare, including hospitalization, dietary changes, and close monitoring. Still, if a gastropexy is performed, recurrence rates drop to under 5%.

📉 Why It’s Worth It

One study found that gastropexy is a highly cost-effective way to protect at-risk dogs from GDV. It’s especially recommended for Great Danes and other high-risk breeds before age one.
